T-Mobile Assurant Claim: How to File for a Lost, Stolen or Damaged Device

T-Mobile customers who pay for device protection may be able to file an Assurant claim when a covered phone, tablet, wearable, hotspot, router, or modem is lost, stolen, accidentally damaged, or affected by a covered mechanical failure. The available remedy may be a repair or a replacement device, depending on the plan, type of damage, model, and service availability.

Before filing, customers should confirm that protection was active on the affected line when the incident happened. Buying coverage after a phone is already damaged or missing will not make the earlier loss eligible. The exact coverage document for the account controls the claim.

T-Mobile

Protection 360 and Standard Device Protection

T-Mobile offers more than one protection plan, and the claim limits are different. Under the current Protection 360 program, accidental-damage claims and mechanical or electrical failure claims do not have a stated numerical limit. Loss and theft coverage allows up to five claims in a rolling 12-month period, measured from the date of the first replacement.

Standard Device Protection is more limited. It provides unlimited claims for covered mechanical or electrical failure, but accidental damage and loss or theft are generally limited to one claim in a rolling 12-month period. New York plans and Bring Your Own Device coverage can have different documents, so customers should check the plan shown on their account rather than relying on a general advertisement.

What Types of Incidents May Be Covered?

Coverage may apply to drops, cracked screens, liquid damage, loss, theft, and covered mechanical or electrical breakdowns. Protection 360 currently advertises $0 front-screen repair for eligible smartphones when repair is available through an Assurant-authorized center. Eligible back-glass repair may also carry a low or $0 service fee, depending on the device tier and current program terms.

Not every damaged phone qualifies for a repair. Assurant may offer an in-person repair, an advanced replacement by mail, Apple service for an eligible Apple device, or another option available under the plan. Cosmetic damage that does not affect operation, intentional damage, pre-existing problems, unauthorized repairs, and other listed exclusions may not be covered.

Information Needed to Start a Claim

Only an authorized person on the T-Mobile account may file the claim. The claimant should have the affected 10-digit mobile number or the device’s 15-digit IMEI, the make and model, the date of the incident, and a clear explanation of what happened. The IMEI may appear on the original box, purchase receipt, T-Mobile account, or T-Life app. On a working phone, it can often be displayed by dialing *#06#.

Assurant may also request the primary account holder’s details, T-Mobile PIN or account login, email address, government-issued identification, proof-of-loss documents, and a domestic U.S. shipping address. Replacement devices cannot be shipped to a P.O. box. A credit card, debit card, or eCheck may be needed to pay the applicable service fee or deductible.

How to File a T-Mobile Assurant Claim

The claim can be started online at MyTMoClaim.com. Customers may also use T-Life, the Protection 360 app by Assurant, or call Assurant at 1-866-866-6285. The online system asks for the mobile number or IMEI, verifies the account, collects incident details, and displays the available repair or replacement options.

For a lost or stolen device, contact T-Mobile promptly to suspend service and reduce the risk of unauthorized use or charges. Assurant’s claim-help page lists 1-877-778-2106 for this purpose. Customers may also be required to disable Find My iPhone, Find My Device, or another security feature before a claim can be completed or a damaged phone can be returned.

Answer every question accurately. Assurant may request additional documents or an identity scan before approving the claim. Missing or inconsistent information can delay review. Customers should file promptly because the allowed reporting period may vary by plan and state.

How Much Is the Deductible?

A non-refundable service fee or deductible may apply to each approved claim. The amount depends on the protection plan, device tier, and whether the claim involves screen repair, other accidental damage, loss, theft, or mechanical failure. Eligible Protection 360 screen repairs may cost $0, while replacement claims for expensive devices can carry substantially higher deductibles.

Customers should use the My Deductible tool on the official claim website for the exact amount assigned to their device. The deductible is generally collected after approval. Taxes may also apply. The monthly protection charge does not replace the deductible; they are separate costs.

What Replacement Device Will You Receive?

An approved replacement is normally a reconditioned device of like type and quality. If a suitable reconditioned unit is unavailable, Assurant may provide a new device of like type and quality. The replacement is not guaranteed to be the same color, and accessories are not automatically included unless the coverage terms apply to them.

Claim approval may be immediate or may take several days when additional verification is required. After approval, Assurant says replacement devices are generally shipped overnight, where available, for delivery within one or two business days. Most Assurant replacement devices include a six-month warranty. Different warranty terms apply to certain Apple Certified replacements.

Returning the Damaged Device

When Assurant sends a replacement for a damaged or malfunctioning device, the claimed device becomes the property of the coverage provider and must be returned. Prepaid shipping materials and instructions are normally included with the replacement.

The return must be completed within 10 days after receiving the replacement. Failure to return it can result in an unrecovered equipment fee equal to the retail price of the replacement device plus applicable shipping costs. Before returning a phone, back up personal data, remove account locks as directed, erase the device, keep the shipping receipt, and save the tracking number until Assurant confirms delivery.

Tracking or Disputing a Claim

Claim status can be checked at MyTMoClaim.com using the mobile number or IMEI and ZIP code. Customers who provide an email address should receive status and shipping updates. If a claim is delayed, review the portal for requests for identification, payment, or additional documents.

If a claim is denied, ask for the specific reason and compare it with the account’s coverage documents. Correct any mistaken incident date, IMEI, account detail, or documentation. Keep copies of the claim number, communications, payment confirmation, return tracking, and photographs of the damaged device. These records are useful if the decision or a nonreturn charge needs to be reviewed.
